The local council assesses eligibility and approves the work. Once agreed, we can manage the adaptation and installation.

Hospital discharge support
Our Home from Hospital service helps patients return to a home which is safe, suitable and warm. We can carry out free home safety checks and arrange work once someone is home, to reduce the risk of readmission. We take referrals at any point in the patient journey – the sooner the better – and typically respond the same day.

How funding works
Adaptations to help someone return home safely are usually funded through the local council. We work with you and the council to move quickly for a timely, safe discharge.
Disabled Facilities Grant (DFG)
Larger adaptations may be funded through a DFG, assessed by the local council. We can refer for assessment where needed.
Fast-track for discharge
For time-critical cases we co-ordinate with the council and NHS partners to get the home ready as quickly as possible.

Referrals can be made by healthcare professionals and hospitals, and we also accept self-referrals and referrals from family members. Complete the online form with the patient, or call us if you would like to talk it through first.
