When someone’s health or mobility changes, their home may need to change too. We work with healthcare and community teams on home assessments, adaptations and discharge support.

Grab rails, access changes and bathroom adaptations can support someone whose home has become difficult to use.
Explore home adaptationsWe can discuss practical changes such as lighting, thresholds and non-slip surfaces, alongside the person’s assessed needs.
For people living with dementia, changes may include consistent lighting, clear routes and visual contrast. Work for someone with a learning disability is considered with the person and their support team.

We work with hospital discharge teams, occupational therapists and community teams when work at home is needed, and referrals can be discussed at any point in the patient journey.
If timing is urgent, please also call 0300 323 0700 during our phone hours: 08:00–16:30, Monday–Friday.
Hospital discharge supportContact us with the person’s location, the difficulties they face at home and any relevant assessment. An occupational therapist’s specification can accompany the referral.
Let us know about a planned discharge or other time pressures. We’ll confirm the service available, funding arrangements and proposed scope before work goes ahead.
We agree how work will be checked and what completion information you need. Referral routes and response times depend on the service agreement.
